The AGES Senior Health Care Department offers
comprehensive health care with a team approach that includes physicians,
dietitian, nurse and social worker in the home, hospital, clinic,
nursing home or assisted living facility
Our primary goals are:
To promote optimum health,
well-being and independence of older persons.
To assist in the prevention or management
of age-related illnesses or chronic disabling conditions.
To facilitate the involvement of older
adults in their own health care decisions.
