The AGES Senior Health Care Department offers comprehensive health care with a team approach that includes physicians, dietitian, nurse and social worker in the home, hospital, clinic, nursing home or assisted living facility

	To assist in the prevention or management of age-related illnesses or chronic disabling conditions.
