Home Care Physicians is a network of experienced, board certified physician, highly qualified medical assistants and support staff that are devoted to providing the highest quality and most advanced medical care to the homebound patient. Home Care Physicians is a division of the Ambulatory Geriatric Evaluation Service (AGES) at Synergy Medical

Home Care Physicians can coordinate with all health care services and providers so that we can offer our patients the most comprehensive in-home patient care.

We can provide the following in-home services:

	Physician visits
	Laboratory services
	EKGs
	Pulmonary function & pulse oximetry

Insurances listed below can be billed for our services. However, an individual should contact their insurance company to see if our services are covered.

	Blue Cross
	Medicaid
	Commercial insurance
