AGES stands
for Ambulatory Geriatric Evaluation Service.
The vision behind AGES is to help our older population seek alternatives.
We believe that growing older does not mean losing one's dignity,
home or sense of community.
Assessment is the key focus of the AGES team, which consists of a physician,
nurse, physical therapist, social worker and dietitian. The AGES team works together
to provide patients with a comprehensive medical assessment. The assessment process
includes looking closely at physical ailments or concerns. However, with our
team approach, emotional, social and community issues also are addressed. There
are many options available to help seniors remain in their homes and an active
part of the community.
The AGES team will point out alternatives, recommend changes in lifestyle and
pinpoint excellent resources in the community that can help you or your loved
one.
Growing older often means different physical conditions and needs. You may want
to think about referral or making an appointment with AGES if there is a dramatic
change in coping with the activities in daily living, or if there are signs of
confusion, forgetfulness or disorientation.
Once the AGES team makes its assessment, it discusses its findings and recommendations
with the patient and the patient's family and/or caregivers. The team also forwards
a copy of these recommendations to the primary care physician.

County residents due to grant support from the Region VII Area Agency
on Aging.
